I have stored all the medical records from my OHNI practice (2005-2009) in a secure location.

In response a request from your physician I will retrieve the file and send by fax the pertinent records from your prior visits to see me. Most of my records are handwritten.

In order for me to release your records you must contact me by phone, letter, or fax. I will call you back at the number stored in your medical records to verify it is you before releasing the records. This is especially important to insure no medical records are released to anyone without their explicit authority.
